Fosters must be at least 24 years old.
Any current pets must be up to date on vaccinations and be spayed/neutered. Exceptions to this will only be considered if proof of a medical explanation is provided.
If you currently rent, you must provide contact information for your landlord or management company in order to confirm any restrictions.
Applicants must live within either the Chicagoland or Southeast Wisconsin/Northeast Illinois areas if interested in fostering. Exceptions will be considered on a case-by-case basis.
Applicants must allow an ALIVE representative into your home for a visit to ensure that your home is safe for the potential new foster animal. All members of the household are required to be present for the visit.
If you've been approved to adopt from us, you've already completed the foster application!

FOSTER APPLICATION
Because we currently do not have a physical location to house animals, we are dependent upon available foster homes to allow us to take new animals into our care. The more foster homes we have in our network, the more animals we can save! If you are interested in becoming a foster, click one of the buttons below to get the process started.
